process to manage the delivery of healthcare to patients with complex
and/or high-cost conditions for both the public and private
sectors. Our highly skilled case managers employ
a longitudinal management approach to coordinate services via a
multi-disciplinary team of providers across the continuum of care.
This approach helps to improve quality of life for the patient,
often allowing them to receive care at home rather than as an inpatient
or in a long-term care setting. At the same time, it maximizes the
