CIMRO utilizes a coordinated, multi-disciplinary process to manage the delivery of healthcare to patients with complex and/or high-cost conditions for both the public and private sectors. Our highly skilled case managers employ a longitudinal management approach to coordinate services via a multi-disciplinary team of providers across the continuum of care. This approach helps to improve quality of life for the patient, often allowing them to receive care at home rather than as an inpatient or in a long-term care setting. At the same time, it maximizes the cost-effectiveness of care. 

Case managers endeavor to ensure case management clients have the necessary education and tools to successfully achieve goals toward better health.  CIMRO's case management services have had a positive effect on the lives of participants with whom they interact.  Supportive of this statement is data from a recent reporting period.  Compared to each enrollee's baseline quarter, there was a 39% decrease in hospitalizations, a 34% decrease in ED visits, a  10% decrease in PMPM Medical Paid, and a 13% increase in overall client compliance.  Additionally, results from a recent annual satisfaction survey revealed that 98% of clients rated the case management program and services as Excellent or Good.
